Follow-Up Rounding

Scheduled follow-up visits and medication management by a nurse practitioner assigned to your building.

Change in Condition

Real-time response from the on-site NP during the day and an on-call clinician overnight, to treat in place where appropriate.

Our Model

Weekday

Mon–Fri · 8a–5p

Each NP is on-site at their assigned facilities, handling day coverage and real-time change-in-condition calls. The physician sees new admits and complex cases.

Weeknight

On-call rotates among the pod's three NPs and the physician. Telehealth-enabled, with physician escalation within 30 minutes.

Weekend

Fri 5p–Mon 8a

One pod member covers on-call. Inpatient rehab sites are rounded in person Saturday and Sunday; new SNF admissions are still seen over the weekend.

Meetings & Cadence

We are hiring internal medicine physicians to join the founding clinical team. You will serve as the attending physician for patients at one or more assigned skilled nursing and rehabilitation facilities, working alongside our social work team. This is a founding-team role: you will help shape rounding workflows, documentation standards, and quality metrics rather than inherit someone else's.

Responsibilities

  • Serve as attending physician for post-acute patients at assigned skilled nursing and rehabilitation facilities.
  • See each new admission and complete medication reconciliation within 48 hours, including calls to the discharging hospital physician and the patient's family.
  • Manage acute and chronic medical conditions in the post-acute setting, with an emphasis on avoiding preventable readmissions.
  • Participate in care plan meetings and discharge planning alongside our social work team and facility staff.
  • Own and transmit SBAR handoffs when a patient is transferred back to the hospital.
  • Coordinate with facility administrators, directors of nursing, and therapy teams as the clinical face of Anchor at your buildings.
  • Maintain timely, accurate documentation and billing in our EMR.

Schedule & Call

  • Week on, week off.
  • During your week on: daily rounding at inpatient rehabilitation, and rounding at skilled nursing facilities three days a week.
  • You cover call for the group during your week on. Call is taken over the phone; there is no in-house call.

Required Qualifications

  • MD or DO with completed residency in Internal Medicine or Family Medicine.
  • Active, unrestricted Texas medical license (or license eligibility with application in progress) and active DEA registration.
  • Board certified or board eligible.
  • Comfort managing medically complex patients with limited on-site ancillary support.
  • Reliable transportation and willingness to travel between assigned facilities in the Metroplex.

Preferred Qualifications

  • Prior experience as a SNFist, post-acute attending, or hospitalist.
  • Familiarity with SNF regulatory requirements, PDPM, and care plan meeting workflows.
  • Interest in a founding-team role with input on clinical standards and practice growth.

Why This Role

  • Week on, week off. Every other week is entirely yours. Call during your week on is phone only, with no in-house call.
  • A real support structure. A dedicated social work team within the same group, not a solo practitioner arrangement.
  • Physician-owned and physician-led. Decisions are made by practicing physicians, and clinical standards are set by the people doing the work.
  • Ground-floor opportunity. Joining at launch with room to grow into leadership as the practice adds facilities.

Compensation & Benefits

  • $275,000+ base salary, plus production incentives.
  • Benefits include health insurance, 401(k), malpractice coverage, and CME support.

Apply with your CV. Qualified candidates will be contacted by one of the founding partners to schedule an initial conversation.
